Platinum membership materials include:  Portfolio with Directory of Benefits (Vacation Guide)  Membership Card and membership materials.

Blue membership materials include:  Portfolio with Directory of Benefits (Vacation Guide)  Membership Card and membership materials.

By submitting this Enrollment Request Form, member indicates that he/she has read and understands the terms and conditions of the Condominium Travel Club, Inc. (CTC) BLUE or PLATINUM membership(s), which agreement(s) are incorporated herein by reference.  I understand my membership will be activated and I will be shipped (at no additional expense) via UPS ground or USPS, my membership materials which include a membership card (www.condominiumtravelclub.com membership login number) 5 business days from receipt of  complete Enrollment Request Form at the offices of CTC.   

I understand my dues are paid for one year and there are yearly membership dues described above, payable to remain a member of CTC. I hereby pre-authorize Condominium Travel Club, Inc. (CTC) to initiate debit entries to my credit card account indicated below to pay Annual Dues as stated above for continued membership in CTC BLUE or PLATINUM membership(s).  On an annual basis notice will be sent (The “Annual Renewal Statement”) prior to transfer of funds, alerting me to the expiration of membership and the automatic payment of Annual Dues.  If my email is provided, notice will be sent electronically. This authorization will remain in full force and affect until CTC has received written notification of its termination in such time and in such manner as to afford CTC a reasonable opportunity to act on it. Annual Dues for the second year of membership are guaranteed at the above amount.  Subsequent years’ annual dues may be increased only as outlined in the terms and conditions of membership. 

Member agrees to pay all expenses incurred for membership, annual dues or travel by check, credit card or money order in US funds.

REFUND POLICY:

Payments for Membership:  Once funds have cleared, refunds will be given within 30 (thirty) days from the date payment is made. Please allow 30 to 60 days to process refund. 

Payments for Annual Dues or Annual Subscription Services:  Members may cancel membership and automatic payment option at anytime prior to or upon receipt of the Annual Renewal Statement.
